I hereby certify that the information provided above together with the supporting documents are true in all respects. I shall be liable for any damage caused to the Group if the information or documents that I have provided are incorrect or not true.

Please read and understand the remarks below before filling out this form.

(1) You must clearly and sufficiently specify the details of the personal data that you wish to exercise your rights so that the Group can proceed with your request accordingly. If you do not provide sufficient details to process your request, the Group may not be able to process your request. The Group will request the supporting documents for the purpose of verifying your identity for exercising your rights i.e. a copy of your ID card or a copy of passport or documents giving authority together with a copy of the data subject’s ID card in case of grantee.

(2) The Group will use the personal data you have provided in this form for processing your request. The Group will verify the accuracy first and may send your request form to its affiliates, contractors or service providers in order to process your request. Your request will be completed within a reasonable period of time as the case may be but not later than 30 days from the date of receiving the request. However, the Group will not proceed with your request if such processing infringes the privacy of another person or is against the law.

(3) In the case of a request for withdrawal of consent, the Group will restrict the collection, use and disclosure of your personal data which is processed on your consent basis and the Group will notify its agent or data processor to restrict such processing under the Personal Data Protection Act. However, such withdrawal shall not affect the collection, use or disclosure of your personal data which was prior processed on your given consent basis.

(4) Withdrawing your consent may prevent you from receiving the news or information about products and services of the Group.

(5) The Group may charge a fee if your request appears to be lavish or unfounded.
